This form is required and should be filled out after completing the Baptism Class. Once this form is filled out, you will be contacted by Jared Harms who will set up a time to film your testimony to be shown during your baptism.

Baptism Video Interviews
Jared will shoot a quick 30-second video shown before you get baptized here are the questions you will be asked:
1) I am getting baptized this morning because?
2) To me Jesus is_________?
3) Before Jesus was my Savior I was (my life was) ____________?
4) Today, (Who is baptizing you ) is baptizing me because _________?
5) How did you start following Jesus? 6) And finally, who you would like to thank (i.e. who lead you to Christ, your life group, youth group,
etc..)
** Please write your response down on a sheet of paper. Remember, we are shooting a short 30-45 second video so your responses will need to be 1-2 sentences. When you come in, we will review this with you and then shoot your video.
When scheduling for a video shoot, we ask that each person allow 45 minutes in their schedule for the shoot.

FOR KIDS
Many children don’t have many details to offer, “what life was like before Christ” so we usually begin with question 2 (above) and encourage the children to share a memorized scripture verse when possible. PLEASE PREPARE A VERSE BESIDES JOHN 3:16 ☺
